1. How do I obtain my lab tests and results?

If you are a new patient and requested to have a follow-up appointment, Drs. Guerin or Mafong will explain your results to you in-person at that appointment.  For routine ongoing problems, a medical assistant will call you with the results. If you are expecting a call and do not hear from us within 1 week, please call our office.

 2. Should I obtain my labs before my office visit?             

No, appropriate lab tests will be obtained, if necessary, after a thorough evaluation by one of our physicians.   If you already have lab results from your primary provide provider, please bring those results to your appointment. 

3. How do I obtain my records from my current doctor for your office?

Please call your health care provider and advise them what records you need.  They may either be released to you directly or faxed to our office at 760-941-9845. 

4. How do I obtain my records from your office?

Please fill out the following Record Release Form (to request a copy of your records from us)  download the form here.  It is helpful to ask for specific items, rather than the entire chart.  Please allow at least 48 working hours for the records.

 5. How do I obtain prescription refills?

Please have your pharmacy fax us the request during regularly scheduled hours. Please allow 24-48 hours for these to be processed.  Unfortunately, we cannot refill medications on weekends or after hours.

  6. Do you have an appointment cancellation policy?                     

If you need to cancel, please give us at least 24 hours notice to avoid any charge.  We reserve a significant amount of time so that we can fully address your medical needs.  We reserve the right to charge for a consultation or an office visit if you do not keep your appointment. 

7. Do you have a policy in regards to phone consultations?

Providing high quality health care with accurate diagnosis and treatment plans requires face-to-face contact.

If you are an established patient and feel your problem is urgent, please call our office and all efforts will be made to accommodate you into the schedule.

8. Will your office bill my insurance?

Yes, of course.  We will bill Medicare (if you qualify) and one additional insurance as a courtesy.  Since your insurance is a contract between you and your insurance company, you are responsible for the cost of services you receive.  Co-pays and deductibles are due at the time of service.  There is a $15.00 dollar fee for all returned checks

If you have a question regarding your bill, please call the billing department. 

Please note, it is the responsibility of the patient to determine if you need authorization for any services, procedures or visits.  In addition, it is the patient’s ultimate responsibility to be sure the lab still has a current contract with your insurance company. This can change frequently without notice.

9.  How can I get disability and other forms filled out?

It is best to bring the forms to your office visit.  Often, they can be filled out at the time of the visit if they are simple.  It is helpful to fill out as much of the form as possible prior to your visit.
