1. Call for an
appointmentPlease call our office at 760-941-9850
during regular Office Hours (8am- 4pm
Mon-Fri) to schedule an appointment.
2. Complete medical
questionnaire
Please download
and complete the following sign – in form
and medical questionnaire PRIOR to your
visit. All new patients and
patients who have not been seen in over
2 years should complete this form.

3. Office co-pays
and deductiblesThese are due at the time of service.
We accept cash or personal checks.
Unfortunately, we are unable to process
credit or debit cards at this time. For
any questions regarding billing, please
contact Karen Schropp at 760-945-9418
between the hours of 9 am – 3pm.
4. Insurance
coverageWe will bill Medicare (if you qualify)
and one additional insurance as a
courtesy. However, the patient is
ultimately responsible for all bills.
If there are extenuating circumstances,
please call the billing department to
discuss payment options.
